As the parent or legal guardian of a minor student, I hereby authorize any member of SnowSports Northwest, or its appointee, to transport the minor student to the most accessible medical facility or hospital, and further authorize any qualified physician to provide any necessary care to the minor student deemed essential to the minor's health or well-being. In the event surgery is necessary, I authorize any licensed surgeon to perform surgery that is deemed necessary by two licensed physicians.
I recognize that skiing/snowboarding is a hazardous sport that can result in serious injury (or death). I accept the risks inherent in skiing/snowboarding and in the ski area/mountain environment. I agree to Release, Forever Discharge, Defend, Indemnify , and Hold Harmless SnowSports Northwest, Ski Lifts, Inc., DBA The Summit at Snoqualmie, its parent company, subsidiaries and affiliates, The U.S. Government, their respective agents, officers, distributors, and all event sponsors (hereinafter "Releases") of and from any and all liability, claims, lines, demands, actions, causes, of action whatsoever arising out of or related to any loss, cost, damage or injury, including death, of any person or damage to property of any kind related in any way to the Program's operations or the errors or omissions of the Program, its agents, subcontractors, volunteers, or any other person directly or indirectly employed by them, or any of them, while engaged in any activity associated with this agreement whether contractually or otherwise.
If I am signing on behalf of a minor, I recognize that I may not release any claims the minor may have. However, I accept full responsibility for all medical expenses incurred as a result of the minor's participation in or travel to and from Summit Central a Snoqualmie Pass. I also agree to RELEASE, HOLD HARMLESS, and INDEMNIFY SnowSports Northwest, and Ski Lifts, Inc. DBA The Summit at Snoqualmie for any claims brought by the minor.
As the parent or legal guardian, I permit the student to participate SnowSports Northwest programs, am aware of the refund policy, have accident insurance, will abide by the programs rules, and agree not to hold SnowSports Northwest or its agents liable for any accident, damages or loss of equipment which may be sustained as a result of participation in this program.
I have carefully read this release of liability, fully understand its contents, and sign it of my own free will.
